I am not at all surprised at this occurrence. Recently I have encountered a similar problem with this company. Not just once. It occurred three times. (This last time it was with a serious drug that could have caused my death) I reported to the head director over the southeastern districts pharmacies. I had hoped to prevent this sort of thing from happening. (As a footnote...I switched pharmacies to an old school pharmacist.)
My advice:
Most of the time your doctor or nurse will explain to you the medications they prescribe. They will explain how they want you to take your medications. Write this down. When you pick up your prescription from the pharmacist check to make sure the instructions match. If they do not match then question the pharmacist. Be proactive with your own health. Unfortunately in this case nothing could be done to prevent this except per the pharmacist.

I am sick of this "America/The British are horrible" campaign. The fact is that many Iraqis have fled their country. They wish for soldiers to bring peace to their country. (I wish we would let them have it. Why waste more American lives on something that should have never happened in the first place.) The fact is that this is your average Americans/Brits that have joined this endeavor did so with pure intentions. Those that are over there are for the most part outstanding human beings. There will always be that disturbed segment of people in any society. Acts of barbarism are wrong regardless of the origin. I hope this did not happen. At this point it is allegations. Where is the balance here? Where are the acts of heroism? We see none. These reports are regarded as propaganda. I don't think many will say they are in agreement with this war. (I have been against it from the beginning) Making our upstanding citizens look foolish and demonic is not accomplishing anything. We are a society of caring people. These men and women are willing to put their lives on the line for the belief they can make a difference in others lives. Perhaps it is a false assumption that you can make a difference in a country that had little regard for human rights prior to this war.....

Verizon, Google, Cisco, and HP are among the companies that have joined a secretive group called the Allied Security Trust.
This is the beginning of patent wars.
This scenario is becoming more and more common... Companies are now launching businesses based on buying patents and using them to sue others.
In order to combat such companies, Verizon, Cisco, Google, Telefon AB L.M., Ericssson, HP and other companies have joined together to launch Allied Security Trust -- an organization which will acquire patents and subsequently grant member companies nonexclusive rights to use them.
A quarter million dollars or so will grant you access to this exclusive patent club and another $5 million is required to be placed in escrow to help fund future patent purchases.
The patent case that sent shivers through the tech industry was BlackBerry maker Research in Motion Ltd.'s $612.5 million settlement in 2006. This is most likely the catalyst for this action.
